Report a Concern

The RESPONDER Partnership is committed to maintaining a safe, respectful, and accountable environment. This form provides a confidential channel for raising concerns about conduct connected to the Partnership, including suspected misconduct, breaches of the Code of Conduct, or other actions that may require review.

When to use this form

You may use this form to report concerns relating to:

  • suspected misconduct or unethical behaviour

  • a breach of the RESPONDER Partnership Code of Conduct or related policies

  • harassment, discrimination, exploitation, abuse, or safeguarding concerns

  • retaliation connected to raising a concern

  • other conduct that may compromise the safety, integrity, or accountability of the Partnership.

Confidentiality and anonymous reporting

Reports may be submitted anonymously. Any identifying information you choose to provide will be handled confidentially and shared only as necessary for appropriate review and follow-up. Further information about confidentiality and the reporting process is provided within the form.

This form is not an emergency service. If you or someone else is in immediate danger, contact the appropriate local emergency service or relevant authority.
